If you have a previous hearing test you are welcome to fax it in advance of your visit.
Please bring in a medical clearance for hearing aid evaluation. Alternatively you can have your physician fax or mail us a note specifically stating that you “are medically cleared for use of a hearing aid.”
Please bring any previously available hearing tests. We also perform tests in our office when appropriate.
Please bring your hearing aids even if they are not working.
Please bring a current list of medications and medication allergies.
Please bring any relevant medical records.
Please bring in your insurance card. |
